You may then print the form, and follow the instructions on the form. The cost to order by mail is $17.00, shipping and handling included. To order by credit card, click on the PayPal logo. You will be directed to a secure website, where you can enter credit card information and shipping information. The cost to order electronically is $18.00, shipping and handling included.
